Description: 

Strathspey Railway Company Limited have failed to ensure the safety of staff, volunteers, passengers and other persons affected by their undertaking, so far as is reasonably practicable, in that they failed to effectively manage the risk arising from a change to their standard operating practices during a special event, namely operation of the locomotive 'Flying  Scotsman', including differences in the characteristics of the locomotive used and the management of access to the driving cab.

Description:

Network Rail have failed to implement, so far as is reasonably practicable, effective measures to prevent risks to health and safety of passengers (and other persons at the station) during passenger surges and overcrowding events at London Euston Station.

ORR has agreed with Network Rail to a short timescale extension to the Improvement Notice which allows the company to put the necessary control measures in place.

Description:

Tanfield Railway Company have failed to ensure, so far as is reasonably practicable, that persons employed by them and any other person affected by their undertaking, are not exposed to risks to their health and safety, in that they have failed to undertake a suitable and sufficient assessment of the risks associated with the use of the recently refurbished locomotive shed at Marley Hill, including work undertaken at height.

Description: 

Transport for Wales Limited as the Train Operator have failed to ensure so far as is reasonably practicable that passengers and employees are not exposed to the risk of harm. Three fires have occurred within a one month period between February and March 2023 onboard class 175 trains whilst in passenger service. The operator has failed to implement effective arrangements for the organisation, control and monitoring for the maintenance of the class 175 fleet needed to ensure the safe operation of the transport system.
